Before Your First Oral Surgery Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Fill out and email or print the new patient registration information by clicking on the “Form” link on our website prior to your visit.
  • Your surgical referral slip and any x-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring your cards with you. Complete all insurance information on the patient registration forms prior to your visit –  this will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

 

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. You will also need an adult to drive you home.

 

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., a history of radiation to the head and neck, diabetes, artificial heart valves and joints, etc.), if you require antibiotic premedication (for prior heart valve surgery or joint replacement surgery), or if you are currently taking any medications (i.e., osteoporosis medications, antiplatelet therapy, anticoagulant therapy, etc.).

 

X-RAYS

If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.

Many insurances have different sub-plans, HMO, Medicare and Medicaid options – it is best to have our office verify YOUR SPECIFIC PLAN prior to seeing us! Just because your insurance carrier is listed here does not mean that we participate with your plan,

*While our office participates with many insurance plans it is always best to verify the coverage for your specific plan with the office and/or your insurance provider(s)! Also please note that we will check both Dental and Medical insurances if applicable for your individual treatment plan.

**Kail and Moore Oral Surgery is not a Medical Medicare Provider, however we do participate with several Medicare Dental supplements. Biopsies and certain services that are classified as medical would not be covered at our facility because of our Medicare status (this includes medical Medicare supplements).

Financing can be arranged through the office for certain procedures

 

PLEASE NOTE: PAYMENT IS DUE AT THE TIME OF SERVICE. Certain procedures will require a deposit in order to schedule in order to cover the cost of custom items (surgical guides, custom implants, etc.)

 

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated.
